Overview

Vitamin toxicity (hypervitaminosis) is uncommon from food alone but can occur with high-dose supplementation. Fat-soluble vitamins (A, D, E, K) are stored in body fat and the liver, so they can accumulate and carry more risk of toxicity with sustained excess intake. Water-soluble vitamins (B-complex, C) are generally excreted in urine and carry lower toxicity risk overall — with notable exceptions such as vitamin B6 and niacin.

Vitamin A toxicity

Chronic intake well above the tolerable upper intake level can cause headache, liver damage, bone pain, and, in pregnancy, birth defects. This applies to preformed retinol (found in supplements and animal foods), not to beta-carotene from plant foods, which is not associated with toxicity — very high intake can harmlessly discolor the skin.

Vitamin D toxicity

Rare, but possible with very high, sustained supplemental doses. It causes hypercalcemia — nausea, weakness, and, in severe cases, kidney damage. Sun exposure does not cause vitamin D toxicity; it self-regulates in skin.

Vitamin B6 toxicity

Unlike most B vitamins, chronic high-dose B6 supplementation (well above the RDA, typically from long-term supplement use) has been linked to sensory nerve damage — numbness and tingling in the hands and feet — which is usually reversible after stopping the supplement.

Niacin toxicity

High-dose niacin, often used therapeutically for cholesterol management under medical supervision, commonly causes flushing and, at very high sustained doses, can cause liver toxicity. This is distinct from the small amounts of niacin found in food and standard multivitamins.

Vitamin C toxicity

Very safe; the main risk at high doses is gastrointestinal upset (diarrhea, cramps). People with hereditary hemochromatosis should be cautious given vitamin C’s effect on iron absorption.

Safe supplementation principles

  • Prioritize food sources first.
  • Don’t exceed the tolerable upper intake level (UL) without medical supervision.
  • Be especially cautious with fat-soluble vitamins, which accumulate in the body over time.
  • Choose third-party tested products (USP Verified, NSF Certified).
  • Pregnant women, children, and older adults should be more cautious with high-dose supplementation.
