Overview
Milk kefir is a fermented milk drink made by culturing milk with kefir “grains” — gelatinous clusters of bacteria and yeast living in a polysaccharide matrix, originating in the Caucasus Mountains region. Unlike yogurt, which typically uses just a couple of bacterial strains, kefir grains host a notably diverse microbial community (commonly cited as containing dozens of bacterial and yeast species, though exact composition varies by culture and origin). The result is a tangy, slightly effervescent, drinkable fermented milk.
Active compounds and evidence
Kefir’s microbial diversity is well documented analytically, and it is a genuine source of live cultures along with protein, calcium, and B vitamins carried over from milk. Human research on kefir specifically (as opposed to fermented dairy or probiotics generally) is a smaller but growing evidence base; some trials suggest modest benefits for lactose digestion (the fermentation process breaks down some lactose) and gut symptom improvement in people with mild digestive complaints, though results across specific health outcomes like immune function or cholesterol are mixed and not yet strong enough to support broad clinical claims.
Safety and contraindications
- Lactose content: Fermentation reduces but does not eliminate lactose; kefir is often better tolerated than milk by people with mild lactose intolerance, but it is not lactose-free and may still cause symptoms in more sensitive individuals.
- Dairy allergy: Milk kefir is not appropriate for those with a true milk protein allergy (distinct from lactose intolerance).
- Trace alcohol: Like kombucha, kefir fermentation produces a small amount of alcohol (typically well under 1%), relevant for people avoiding alcohol entirely.
- Immunocompromised individuals: As with other live-culture fermented foods, people who are significantly immunocompromised should discuss regular consumption with their clinician.
How it’s typically prepared
Kefir grains are added to milk (dairy or, less traditionally, non-dairy alternatives) and left at room temperature for roughly 24 hours, then strained and the grains reused for the next batch.
