Overview

Irritable bowel syndrome (IBS) is a functional gut disorder — meaning symptoms like abdominal pain, bloating, and altered bowel habits occur without visible structural damage — diagnosed using symptom-based criteria (Rome IV) and subtyped by predominant bowel pattern (IBS-C, IBS-D, IBS-M).

Inflammatory bowel disease (IBD) includes Crohn’s disease (which can affect any part of the GI tract in a patchy pattern) and ulcerative colitis (which affects the colon continuously), both driven by chronic immune-mediated inflammation and typically managed with anti-inflammatory or biologic medications.

GERD (gastroesophageal reflux disease) occurs when stomach acid regularly flows back into the esophagus, often due to a weak lower esophageal sphincter, and is managed with acid-suppressing medication and lifestyle changes.

Celiac disease is an autoimmune condition triggered by gluten (a protein in wheat, barley, and rye), causing immune-mediated damage to the small intestine lining; it is confirmed with blood antibody testing and intestinal biopsy.

Peptic ulcer disease involves sores in the stomach or duodenum lining, most often caused by H. pylori infection or long-term NSAID use.

Diverticular disease involves small pouches (diverticula) in the colon wall, most common in the sigmoid colon; diverticulitis is inflammation or infection of these pouches.

Nutritional Considerations

IBS: The low-FODMAP diet — temporarily restricting fermentable carbohydrates (found in foods like onions, garlic, wheat, certain fruits, and dairy), then systematically reintroducing them — has the strongest evidence base for symptom relief and is best done with guidance from a dietitian trained in the protocol, since long-term unsupervised restriction can affect gut microbiome diversity and nutrient adequacy.

IBD: No single diet treats Crohn’s disease or ulcerative colitis, but nutrition support is important because both conditions raise the risk of malnutrition and specific nutrient deficiencies (iron, vitamin B12, vitamin D). During active flares, a lower-fiber, easier-to-digest diet is often recommended temporarily; during remission, a broader, well-balanced diet is generally encouraged. Dietary changes should be coordinated with the treating gastroenterologist.

GERD: Common, evidence-informed lifestyle measures include avoiding large meals close to bedtime, limiting known trigger foods (which vary by individual but often include caffeine, alcohol, chocolate, and high-fat or spicy foods), maintaining a healthy body weight, and elevating the head of the bed. These support, but don’t replace, medical treatment when needed.

Celiac disease: The only effective treatment is a strict, lifelong gluten-free diet, avoiding wheat, barley, and rye. Oats are controversial due to cross-contamination risk and should generally be certified gluten-free. This diet should not be started before diagnostic testing is complete, since starting early can affect test accuracy — testing should always precede dietary changes here.

Peptic ulcer disease: Treatment centers on eradicating H. pylori infection with prescribed antibiotic regimens and/or stopping NSAID use, both directed by a physician. Avoiding alcohol and smoking supports healing; there’s no strong evidence that bland diets speed ulcer healing, though individuals may find some foods worsen symptoms.

Diverticular disease: A high-fiber diet is generally recommended for diverticulosis prevention and long-term management. During an acute diverticulitis flare, physicians often recommend a temporary low-fiber or clear liquid diet, advancing back to high fiber as symptoms resolve — this should follow the treating physician’s guidance.

Safety

These conditions range from manageable with lifestyle changes to requiring urgent medical care (e.g., a severe diverticulitis flare or IBD flare). Diagnosis should always precede major dietary restriction, particularly for celiac disease. Any elimination diet (low-FODMAP, gluten-free) should ideally be supervised by a registered dietitian to avoid unnecessary long-term restriction or nutrient gaps.
