Overview
Iron deficiency anemia (IDA) is the most common form of anemia worldwide. It develops when the body doesn’t have enough iron to make adequate hemoglobin, the protein in red blood cells that carries oxygen. Common causes include chronic blood loss (heavy menstrual periods, gastrointestinal bleeding, frequent blood donation), inadequate dietary iron intake, increased needs during pregnancy and growth, and poor absorption from conditions like celiac disease or after certain gastric surgeries. Symptoms include fatigue, pale skin, shortness of breath, dizziness, brittle nails, and unusual cravings for non-food items (pica). Diagnosis relies on blood tests, most notably a low serum ferritin, which reflects the body’s stored iron.
Nutritional Considerations
- Heme vs. non-heme iron: Heme iron, found in red meat, poultry, and fish, is absorbed more efficiently than non-heme iron from plant sources such as beans, lentils, tofu, spinach, and iron-fortified cereals.
- Enhancing absorption: Pairing non-heme iron sources with vitamin C-rich foods (citrus, bell peppers, tomatoes) can improve absorption.
- Inhibitors of absorption: Tea, coffee, calcium supplements, and dairy products can reduce iron absorption when consumed at the same time as iron-rich meals or oral iron supplements; spacing them a couple of hours apart is a common practical strategy.
- Oral iron supplements: Ferrous sulfate and similar oral iron salts are typically first-line treatment for confirmed deficiency. Gastrointestinal side effects (constipation, nausea, dark stools) are common and dosing schedules are often adjusted by a clinician to improve tolerance.
- IV iron: Intravenous iron (e.g., ferric carboxymaltose, iron sucrose) may be used when oral iron isn’t tolerated, absorption is impaired, or deficiency is severe. This is administered in a clinical setting under medical supervision.
- Anemia of chronic disease: Some patients have low iron availability due to inflammation (elevated hepcidin) rather than true iron deficiency. Ferritin can appear normal or even elevated in this setting because it is also an inflammatory marker, so interpretation should be done by a clinician alongside other iron studies (transferrin saturation, TIBC). Iron supplementation is not automatically appropriate here and treating the underlying condition is the priority.
Safety & When to See a Doctor
Do not start iron supplements without a confirmed diagnosis — unnecessary iron supplementation can mask other conditions and, in people with hereditary hemochromatosis or other iron-overload disorders, can be harmful. Unexplained iron deficiency in adults (especially without an obvious cause like menstruation or pregnancy) warrants medical evaluation for sources of blood loss, including gastrointestinal causes. Seek prompt medical care for chest pain, severe shortness of breath, fainting, or signs of significant bleeding.
